Arriver à intégrer une mécanique de « mislead » dans un film qui se veut aussi émotif était l'un des grands défis de réalisation. Ensuite, le tournage en soi était excessivement compliqué. Tourner dans un bloc opératoire à l’intérieur des couloirs stérilisés était risqué. L’opération à cœur ouvert était une vraie opération. Chacun des gestes posés pouvait potentiellement être une question de vie ou de mort pour le patient. Un autre grand défi aura définitivement été la direction du personnel hospitalier, pour le représenter de façon juste et ne pas créer de « clash » avec l’utilisation de comédiens professionnels.
